Settings for OpenVPN in ChromeOS

I'm using a newer version of Chrome that supports setting up an OpenVPN in the GUI. But I need some support figuring out what to put into all the fields. Can anyone help?

Server host name: Where can I find the IP addresses or host names for the servers to input directly into the field?
Pre-shared key: Do I leave this blank?
Server CA certificate: I downloaded the master.zip file but the .crt certificate won't import. Any idea why?
User certificate: None?
Username: Is this where I put my token after it's been calculated?
Password: Blank?
OTP: Blank?

@OP

1. If you're not too fussed about which server to connect to, use linux-balancer.cryptostorm.net. Other servers can be found here - the naming convention of the configuration files follows the host name of the respective exit node, so out of "cstorm_linux-latvia_udp.ovpn", the host name is "linux-latvia". After that, just tack on "cryptostorm.net" or "cstorm.pw" so that the fully qualified domain name for the Latvia node reads linux-latvia.cryptostorm.net or linux-latvia.cstorm.pw.
2. Pre-shared key is blank
3. The certificate file in raw format can be found here.
4. User certificate is none
5. Yes
6. Can be anything, but must be at least one character.
7. OTP is blank.
